Dismissal Forms

In order to help the process remain safe and efficient, it is essential that families fill out accurate dismissal notes with the correct names of who will be picking up each day. Please find the appropriate form below. Thank you!

Typical Dismissal Plan Form

Please fill out this form for every child in your family at the beginning of the year. You should only need to fill this out once for each child at the beginning of the year to communicate your child's typical plan for the end of the day.

Permanent Change Form

Please use this form if your child has a permanent change to their typical routine. For example, if your child originally was taking the bus everyday but will now be picked up every Thursday.

Daily Change Form

Please use this form if your child has a change in their typical dismissal routine for a particular day. If you need to make a change to your child's daily dismissal after the form is closed in the afternoon, please call the main office; 781-934-7680.
